    Vital Score's mobile engagement platform let patients assess their health, identify a health goal, and self refer to services matched to their goal in less than 90 seconds. Vital Score enabled deeper and more productive patient-provider conversations when patients are most receptive, at the point of care. Vital Score improved outcomes and generated revenue for provider networks through referrals to wellness and ancillary programs. Deployment in several health systems showed increased referrals to underutilized services and dramatically more patient engagement in weight loss, smoking cessation, and better self-care goals.

During my tenure we had almost 100% retention of existing customers and doubled the LOB.
